HOYA Technosurgical Co., Ltd.'s Basic Policies for the Protection of Personal Information

1. Approach to the protection of personal information

HOYA Technosurgical collects and uses customer names, addresses, telephone numbers, email addresses and other information to promote its business operations seamlessly. . We are aware that the appropriate protection of the personal information of our customers as well as that of company officers, employees, contract staff,and other employees (personal information) is a serious responsibility of the company,and, to fulfill that responsibility, we handle personal information appropriately and safely based on these policies. The handling of “individual number” and “specific personal information” under the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures shall be set forth separately.

(1) We shall comply with the Law on the Protection of Personal Information and other related laws and ordinances. We shall also ensure our adherence to practices regarding handling personal information, which is generally deemed to be fair and appropriate, and handle that information appropriately. We shall furthermore strive to realize sustained and proper improvements to continue handling such information appropriately and pertinently.
(2) We shall clarify rules regarding handling personal information and strive to familiarize the employees with them thoroughly. In addition, we shall also require subcontractors and other business partners to handle personal information appropriately.
(3)When collecting personal information, we shall specify and notify or release the purpose of use, such as indicated in item 2 below, and handle personal information by the purpose of use.

(4) We shall devise necessary measures and implement appropriate management to prevent personal information divulging, loss, alteration, etc.,.
(5) The disclosure, revision, discontinuation of usage, etc., of personal information shall be accepted by the department that maintains the personal information or that provides service. In addition, inquiries shall, in principle, be limited to those received from the individual in question. The company may also require identity verification.

2. Purpose of use, etc.

The company shall acquire the personal information required for business performance to facilitate operations. Such personal information shall be used for the purposes given below.

In addition, to promote seamless business operations, personal information may be provided to subcontractors to the extent required. In such an event, the company shall enter into contracts with such subcontractors regarding handling and appropriately monitoring personal information.

(1) To provide services in response to requests from customers and engaging in related or incidental conduct (e.g., provision of support to customers relating to the services that are provided).
(2) To introduce customers to the company’s products or services.
(3) To request the provision of opinions or impressions of customers regarding the HOYA Group (including opinions and impressions of products and services).
(4) To prepare classified statistical materials to contribute to future improvements in services.
(5) To provide personal information in response to the demands of courts of law, law and ordinances, etc.
(6) For other purposes specified by the company upon the receipt of information from customers.
The company shall not use personal information other than for the purposes indicated above without the consent of the customer.

HOYA Technosurgical Co., Ltd.'s Basic Policy on the Handling of Individual Numbers and Specific Personal Information

HOYA Technosurgical Corporation (hereinafter referred to as “our company”) will comply with the “Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures” (hereinafter referred to as the “Number Act”) and the “Guidelines for the Proper Handling of Specific Personal Information (for Businesses)”, as well as the “Act on the Protection of Personal Information” (Personal Information Protection Act) and guidelines of each ministry and agency with regard to the handling of individual numbers and specific personal information (hereinafter referred to as “specific personal information, etc.”).

1. Purpose of Use

Our company will use the specific personal information provided for the following purposes:

(1) Specific Personal Information of Business Partners.
Preparation of payment records, such as those related to compensation, fees, and contract payments, as the Income Tax Act prescribes.
(2) Specific Personal Information of Our Employees.
(3) Specific Personal Information of Spouses and Relatives of Our Employees.

  • Preparation of withholding tax certificates
  • Preparation of documents for dependents’ deductions (changes), insurance premium deduction claims, and special spouse deductions for salary earners
  • Preparation of salary payment reports
  • Preparation of notification forms for changes in salary earners related to special withholding
  • Preparation of applications for switching to special withholding
  • Preparation of payment records for retirement allowances and other benefits
  • Preparation of income tax return documents related to retirement income
  • Preparation of documents and notifications related to property accumulation savings and property accumulation pension savings
  • Notification, application, and claim tasks for defined contribution pensions
  • Provision of information to employee stock ownership plans for preparation of payment records for members of such plans
  • Other taxation-related tasks as stipulated in Article 9 of the My Number Act

  • Notification, application, and claim tasks for health insurance and welfare pension insurance
  • Notification tasks for National Pension Third Category
  • Notification, application, claim, and certificate preparation tasks for employment insurance and workers’ compensation insurance
  • Other social insurance-related tasks, as stipulated in Article 9 of the My Number Act

2. Measures for Safety Management

Our company will adhere to the “Personal Number and Specific Personal Information Handling Regulations” established separately by HOYA to prevent leakage, loss, or damage of specific personal information and to ensure the proper management of individual numbers.

3. Handling of Delegated Tasks

Our company may delegate the handling of specific personal information to third parties. In such cases, we will exercise the necessary and appropriate supervision of the delegated parties by the My Number Act and the Personal Information Protection Act.

 

4. Continuous Improvement

Our company strives to  improve the continuous handling of specific personal information.

5. Disclosure of Specific Personal Information

When the individual or their representative requests to disclose retained personal data related to specific personal information, our company will respond without delay, except in the following cases:

  • When there is a risk of harm to the individual or a third party’s life, body, property, or other rights and interests.
  • When there is a significant risk of a hindrance to the proper execution of our business.
  • When it would result in a violation of laws or regulations.

Use of Google Analytics

Google’s use of information collected from the HOYA Technosurgical Corporate Site by Google Analytics is governed by the Google Policies and Terms (Technology) site, and Google’s Privacy Policy is governed by the Google For Google’s privacy policy, please refer to the Google Policy and Terms (Privacy Policy) site.
